Hipocalemia.<\/em><\/p>\n\n\n\n<p><strong>Coment\u00e1rio do professor: <\/strong><br>Quest\u00e3o te\u00f3rica sobre cetoacidose diab\u00e9tica abordando dist\u00farbios eletrol\u00edticos que acompanham este quadro. Vamos revisar tamb\u00e9m o tratamento inicial, pois \u00e9 o tema mais abordado nas quest\u00f5es. Na avalia\u00e7\u00e3o da cetoacidose, o primeiro passo \u00e9 definir se h\u00e1 mesmo cetoacidose diab\u00e9tica. Os crit\u00e9rios para o diagn\u00f3stico s\u00e3o:<br>\u00b7 Hiperglicemia (>200mg\/dL);<br>\u00b7 pH venoso &lt;7,3 ou bicarbonato &lt;18 mmol\/L (aten\u00e7\u00e3o para este crit\u00e9rio, que foi atualizado em 2022; o crit\u00e9rio era de 15<br>mmol\/L anteriormente);<br>\u00b7 Cetonemia (beta-hidroxibutirato \u22653mmol\/L) ou moderada ceton\u00faria \u22652+.<br>Para o tratamento, cada servi\u00e7o tem seu protocolo com os pormenores do tratamento, mas a ideia central \u00e9 sempre a mesma.<\/p>\n\n\n\n<p>Na primeira hora de tratamento, vamos proceder com hidrata\u00e7\u00e3o endovenosa. Se o paciente estiver inst\u00e1vel, faremos expans\u00e3o com soro fisiol\u00f3gico 20ml\/kg r\u00e1pido. Caso contr\u00e1rio, faremos hidrata\u00e7\u00e3o com soro fisiol\u00f3gico 10-20ml\/kg em 20-30 minutos, at\u00e9<br>restaurar a circula\u00e7\u00e3o perif\u00e9rica. A ideia \u00e9 n\u00e3o fazer uma hidrata\u00e7\u00e3o excessiva, para evitar o edema cerebral.<br>Apenas ap\u00f3s a primeira hora de tratamento iremos iniciar a insulina.
