desequil\u00edbrio pode levar a consequ\u00eancias graves, especialmente cardiovasculares.\u00a0<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-para-que-serve-o-potassio\"><span class=\"ez-toc-section\" id=\"Para_que_serve_o_Potassio\"><\/span><strong>Para que serve o Pot\u00e1ssio?\u00a0<\/strong><span class=\"ez-toc-section-end\"><\/span><\/h2>\n\n\n\n<p>O <strong>pot\u00e1ssio <\/strong>(K+) \u00e9 o <strong>c\u00e1tion mais abundante no meio intracelular, <\/strong>desempenhando um papel crucial na <strong>manuten\u00e7\u00e3o do potencial el\u00e9trico da membrana celular<\/strong>. Seus n\u00edveis normais no meio extracelular variam entre <strong>3,5 e 5 mEq\/L<\/strong>. A despolariza\u00e7\u00e3o celular envolve o efluxo de pot\u00e1ssio, <strong>alterando a carga da membrana e disparando o potencial el\u00e9trico.<\/strong> Para a repolariza\u00e7\u00e3o, a <strong>bomba de Na+\/K+ ATPase<\/strong> atua ativamente, transportando esses \u00edons de volta para dentro da c\u00e9lula, um processo essencial para a excitabilidade neuromuscular e card\u00edaca.<\/p>\n\n\n\n<p>Dentro da regula\u00e7\u00e3o desse \u00edon no corpo, temos duas importantes an\u00e1lises: <strong>Hipocalemia e Hipercalemia.<\/strong><\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-o-que-e-a-hipocalemia\"><span class=\"ez-toc-section\" id=\"O_que_e_a_Hipocalemia\"><\/span><strong>O que \u00e9 a Hipocalemia?<\/strong><span class=\"ez-toc-section-end\"><\/span><\/h2>\n\n\n\n<p>A hipocalemia, definida como <strong>n\u00edveis s\u00e9ricos de pot\u00e1ssio abaixo de 3,5 mEq\/L,<\/strong> \u00e9 um dist\u00farbio eletrol\u00edtico comum com diversas etiologias e manifesta\u00e7\u00f5es.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"h-fisiopatologia-da-hipocalemia\"><span class=\"ez-toc-section\" id=\"Fisiopatologia_da_Hipocalemia\"><\/span><strong>Fisiopatologia da Hipocalemia<\/strong><span class=\"ez-toc-section-end\"><\/span><\/h3>\n\n\n\n<p>A hipocalemia pode resultar de <strong>perdas excessivas de pot\u00e1ssio<\/strong> (gastrointestinais ou renais), <strong>desvio de pot\u00e1ssio para o compartimento intracelular<\/strong>, ou <strong>ingest\u00e3o inadequada<\/strong>. Vale destacar que causas importantes como o uso de <strong>diur\u00e9ticos, hiperaldosteronismo e s\u00edndrome de Liddle, al\u00e9m da s\u00edndrome de realimenta\u00e7\u00e3o.<\/strong><\/p>\n\n\n\n<p>As perdas renais s\u00e3o particularmente complexas de identificar. <strong>Diur\u00e9ticos<\/strong>, como a furosemida (que age na al\u00e7a de Henle, bloqueando o <strong>canal K\/Na\/2Cl,<\/strong> aumentando a <strong>entrega de s\u00f3dio no t\u00fabulo distal e estimulando a aldosterona<\/strong>) e a <strong>hidroclorotiazida<\/strong> (que bloqueia o ENAC no t\u00fabulo distal), promovem <strong>aumento na excre\u00e7\u00e3o de pot\u00e1ssio<\/strong>.\u00a0<\/p>\n\n\n\n<figure class=\"wp-block-image\"><img decoding=\"async\" src=\"https:\/\/lh7-rt.googleusercontent.com\/docsz\/AD_4nXdBQZm51zVR_RwxuCxkFtYhPb0WLjm6YkL4CzMyOTppf5cR7evEFYAte7CwjE-hHp0HWGdnN43UvrI73k8lDNPme5PFfcIq5DlIetwIhrRxKXYatwfkt4tBK26WvZKW9hG2-l_8IA?key=37usbse4kB_JMBGfwVuQMw\" alt=\"\"\/><figcaption class=\"wp-element-caption\">Canal K\/Na\/2Cl presente na Al\u00e7a de Henle, aumentando a excre\u00e7\u00e3o de K+. No <strong>hiperaldosteronismo<\/strong>, o excesso de aldosterona estimula a <strong>reabsor\u00e7\u00e3o de s\u00f3dio<\/strong> no t\u00fabulo coletor, resultando em <strong>espolia\u00e7\u00e3o de pot\u00e1ssio e \u00edons H+,<\/strong> levando a <strong>hipocalemia e alcalose<\/strong>. J\u00e1 na <strong>s\u00edndrome de Liddle<\/strong>, um pseudoaldosteronismo, h\u00e1 um <strong>defeito no canal amilorida-sens\u00edvel<\/strong>, causando <strong>reabsor\u00e7\u00e3o excessiva de s\u00f3dio<\/strong> e <strong>perda de pot\u00e1ssio, mesmo com aldosterona baixa.<\/strong> A avalia\u00e7\u00e3o da rela\u00e7\u00e3o pot\u00e1ssio\/creatinina urin\u00e1ria (> 13 mEq\/g) pode confirmar a perda renal.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"h-manifestacoes-clinicas-e-eletrocardiograficas-da-hipocalemia\"><span class=\"ez-toc-section\" id=\"Manifestacoes_Clinicas_e_Eletrocardiograficas_da_Hipocalemia\"><\/span><strong>Manifesta\u00e7\u00f5es Cl\u00ednicas e Eletrocardiogr\u00e1ficas da Hipocalemia<\/strong><span class=\"ez-toc-section-end\"><\/span><\/h3>\n\n\n\n<p>As manifesta\u00e7\u00f5es cl\u00ednicas variam de <strong>fraqueza muscular e c\u00e3ibras<\/strong> at\u00e9 <strong>rabdomi\u00f3lise<\/strong> e <strong>paralisia peri\u00f3dica hipocal\u00eamica<\/strong>. No entanto, as altera\u00e7\u00f5es eletrocardiogr\u00e1ficas s\u00e3o as mais precoces e importantes. A <strong>infus\u00e3o deve ser lenta,<\/strong> com tempo m\u00e9dio de dilui\u00e7\u00e3o de 1h30min por ampola, preferencialmente em bomba de infus\u00e3o. Em pacientes com restri\u00e7\u00e3o de volume (ex: insufici\u00eancia card\u00edaca, an\u00faricos), o acesso central \u00e9 prefer\u00edvel.<\/p>\n\n\n\n<ol start=\"2\" class=\"wp-block-list\">\n<li>Hipocalemia n\u00e3o grave (K entre 2,5 e 3,5 mEq\/mL)<\/li>\n<\/ol>\n\n\n\n<p>A administra\u00e7\u00e3o <strong>oral<\/strong> de <strong>cloreto de pot\u00e1ssio (xarope 6% ou comprimido de 600 mg)<\/strong> \u00e9 a via preferencial.\u00c9 crucial manter a vigil\u00e2ncia dos n\u00edveis de pot\u00e1ssio, especialmente em pacientes com inj\u00faria renal ou doen\u00e7a renal cr\u00f4nica. \u00c9 preciso ter <strong>cuidado ao repor fosfato de pot\u00e1ssio conjuntamente,<\/strong> a fim de evitar hipercalemia.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-o-que-e-a-hipercalemia\"><span class=\"ez-toc-section\" id=\"O_que_e_a_Hipercalemia\"><\/span><strong>O que \u00e9 a Hipercalemia?\u00a0<\/strong><span class=\"ez-toc-section-end\"><\/span><\/h2>\n\n\n\n<p>A hipercalemia, definida por <strong>n\u00edveis de pot\u00e1ssio acima de 5 mEq\/L,<\/strong> \u00e9 uma emerg\u00eancia m\u00e9dica que pode ser <strong>fatal<\/strong> devido aos seus <strong>efeitos card\u00edacos.<\/strong><\/p>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"h-fisiopatologia-da-hipercalemia\"><span class=\"ez-toc-section\" id=\"Fisiopatologia_da_Hipercalemia\"><\/span><strong>Fisiopatologia da Hipercalemia<\/strong><span class=\"ez-toc-section-end\"><\/span><\/h3>\n\n\n\n<p>As causas da hipercalemia incluem <strong>d\u00e9ficit na excre\u00e7\u00e3o de pot\u00e1ssio, inj\u00faria renal, uso de medicamentos que poupam pot\u00e1ssio<\/strong> (como IECAs, BRA, espironolactona, amilorida, finerenone), lise celular (ex: rabdomi\u00f3lise, lise tumoral) <strong>e acidose metab\u00f3lica<\/strong>. A <strong>insufici\u00eancia renal<\/strong> \u00e9 uma causa prim\u00e1ria, pois impede a elimina\u00e7\u00e3o adequada do pot\u00e1ssio. Ele estabiliza a membrana card\u00edaca, melhorando o padr\u00e3o eletrocardiogr\u00e1fico e protegendo o cora\u00e7\u00e3o dos efeitos da hipercalemia, sem alterar os n\u00edveis s\u00e9ricos de pot\u00e1ssio. Pode ser <strong>readministrado se n\u00e3o houver melhora.<\/strong><\/p>\n\n\n\n<ol start=\"2\" class=\"wp-block-list\">\n<li>Favorecer o influxo de pot\u00e1ssio para o intracelular:<\/li>\n<\/ol>\n\n\n\n<p>&nbsp;&nbsp;&nbsp;&nbsp;Solu\u00e7\u00e3o polarizante: <strong>Insulina Regular 10 UI + Soro Glicosado 10% 500 mL IV.<\/strong> A insulina ativa o canal Glut4, promovendo o <strong>influxo de pot\u00e1ssio<\/strong> e <strong>glicose para dentro da c\u00e9lula<\/strong>. O soro glicosado <strong>previne hipoglicemia<\/strong>. Para pacientes com restri\u00e7\u00e3o de volume, pode-se usar <strong>Soro Glicosado 50% 100 mL em acesso central<\/strong>.<\/p>\n\n\n\n<p>&nbsp;&nbsp;&nbsp;&nbsp;Beta-2 agonista: <strong>Salbutamol 200 mcg (3 jatos) inalat\u00f3rio<\/strong>, que promove o influxo de pot\u00e1ssio para dentro da c\u00e9lula.<\/p>\n\n\n\n<p>&nbsp;&nbsp;&nbsp;&nbsp;Bicarbonato de S\u00f3dio (NaHCO3):&nbsp; <strong>8,4% 1 mL\/Kg IV.<\/strong> Favorece o influxo de pot\u00e1ssio para dentro da c\u00e9lula, mas deve ser evitado em casos de alcalose.<\/p>\n\n\n\n<ol start=\"3\" class=\"wp-block-list\">\n<li>Espoliar pot\u00e1ssio (remover do corpo):<\/li>\n<\/ol>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Diur\u00e9tico:Furosemida (apresenta\u00e7\u00e3o venosa, a\u00e7\u00e3o r\u00e1pida). A dose pode ser dobrada se o paciente j\u00e1 a utilizava.<\/li>\n\n\n\n<li>Resina de troca: Sorcal.
