<strong>preven\u00e7\u00e3o ao HIV<\/strong>: a P<strong>rEP injet\u00e1vel<\/strong>, aplicada a cada dois meses.&nbsp;<\/em><\/p>\n\n\n\n<p>A <strong>Comiss\u00e3o Nacional de Incorpora\u00e7\u00e3o de Tecnologias no SUS (Conitec)<\/strong> vai avaliar a inclus\u00e3o do medicamento com potencial para <strong>aumentar a ades\u00e3o<\/strong> ao tratamento e <strong>reduzir novas infec\u00e7\u00f5es<\/strong>.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-nova-estrategia-nbsp\"><span class=\"ez-toc-section\" id=\"Nova_Estrategia\"><\/span><strong>Nova Estrat\u00e9gia&nbsp;<\/strong><span class=\"ez-toc-section-end\"><\/span><\/h2>\n\n\n\n<p>Atualmente, o <strong>SUS <\/strong>oferece a <strong>PrEP <\/strong>em <strong>comprimidos<\/strong>, que precisam ser tomados <strong>diariamente<\/strong>. <strong>A vers\u00e3o injet\u00e1vel se destaca pela a\u00e7\u00e3o prolongada<\/strong>, j\u00e1 que \u00e9 aplicada por via intramuscular a cada dois meses, o que pode facilitar a ades\u00e3o.&nbsp;<\/p>\n\n\n\n<p>O parecer da <strong>Conitec <\/strong>ser\u00e1 <strong>fundamental <\/strong>para definir se o cabotegravir, princ\u00edpio ativo dessa modalidade, ser\u00e1 incorporado ao sistema p\u00fablico de sa\u00fade.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-historico\"><span class=\"ez-toc-section\" id=\"Historico\"><\/span><strong>Hist\u00f3rico<\/strong><span class=\"ez-toc-section-end\"><\/span><\/h2>\n\n\n\n<p>O cabotegravir foi registrado pela <strong>Ag\u00eancia Nacional de Vigil\u00e2ncia Sanit\u00e1ria (Anvisa) <\/strong>em 2023 e come\u00e7ou a ser vendido no Brasil na rede privada em 2024. Comercializado pela farmac\u00eautica <strong>GSK <\/strong>e distribu\u00eddo pela <strong>Oncoprod<\/strong>, o medicamento chegou ao mercado com valor m\u00e9dio de quatro mil por dose. <\/p>\n\n\n\n<p>Na rede p\u00fablica, <strong>a PrEP oral est\u00e1 dispon\u00edvel desde 2018<\/strong>, com o esquema tenofovir disoproxil + emtricitabina, oferecido gratuitamente. Embora eficaz, esse modelo exige disciplina di\u00e1ria, o que <strong>dificulta a ades\u00e3o para muitas pessoas<\/strong>.&nbsp;<\/p>\n\n\n\n<p>\u00c9 nesse contexto que a<strong> PrEP injet\u00e1vel bimestral representa uma inova\u00e7\u00e3o promissora para o SUS.<\/strong><\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-como-funciona\"><span class=\"ez-toc-section\" id=\"Como_funciona\"><\/span><strong>Como funciona<\/strong><span class=\"ez-toc-section-end\"><\/span><\/h2>\n\n\n\n<p>O cabotegravir pertence \u00e0 classe dos inibidores da transfer\u00eancia de fita integrase (INSTI), impedindo que o DNA do HIV se integre ao material gen\u00e9tico das c\u00e9lulas humanas. O que \u00e9 essencial para a replica\u00e7\u00e3o do v\u00edrus. O esquema de uso come\u00e7a com <strong>duas doses mensais<\/strong> e depois passa para <strong>aplica\u00e7\u00f5es bimestrais de 600 mg por via intramuscular<\/strong>.<\/p>\n\n\n\n<p>A PrEP injet\u00e1vel \u00e9 indicada para adultos e adolescentes a partir de 12 anos, com <strong>peso m\u00ednimo de 35 kg e diagn\u00f3stico negativo para HIV antes do in\u00edcio do tratamento<\/strong>. A Anvisa recomenda a realiza\u00e7\u00e3o de testes antes de cada aplica\u00e7\u00e3o, a fim de evitar resist\u00eancia ao medicamento.<\/p>\n\n\n\n<p>Al\u00e9m do impacto cl\u00ednico, a inje\u00e7\u00e3o ajuda a reduzir a necessidade de consultas frequentes e a carga psicol\u00f3gica de lembrar-se diariamente da medica\u00e7\u00e3o pode ampliar o alcance da preven\u00e7\u00e3o. Especialmente entre popula\u00e7\u00f5es mais vulner\u00e1veis.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-revolucao-da-prevencao-do-hiv\"><span class=\"ez-toc-section\" id=\"Revolucao_da_prevencao_do_HIV\"><\/span><strong>Revolu\u00e7\u00e3o da preven\u00e7\u00e3o do HIV<\/strong><span class=\"ez-toc-section-end\"><\/span><\/h2>\n\n\n\n<p>Segundo o Minist\u00e9rio da Sa\u00fade, <strong>o Brasil registrou cerca de 46,5 mil novos casos de HIV em 2023, e proje\u00e7\u00f5es indicam que at\u00e9 2033 esse n\u00famero pode chegar a 600 mil<\/strong>. Pesquisas sugerem que <strong>a PrEP injet\u00e1vel poderia evitar at\u00e9 385 mil novas infec\u00e7\u00f5es na pr\u00f3xima d\u00e9cada<\/strong>, n\u00e3o por ser mais eficaz do que a vers\u00e3o oral, mas porque facilita a continuidade do tratamento.<\/p>\n\n\n\n<p>Na pr\u00e1tica, <strong>a incorpora\u00e7\u00e3o do cabotegravir ao SUS pode representar uma verdadeira revolu\u00e7\u00e3o na preven\u00e7\u00e3o do HIV. <\/strong>Al\u00e9m de fortalecer a estrat\u00e9gia de preven\u00e7\u00e3o combinada j\u00e1 existente, a medida teria impacto econ\u00f4mico:<strong> ainda que o custo inicial seja elevado, a redu\u00e7\u00e3o do n\u00famero de novos casos diminuiria de forma expressiva os gastos com tratamento ao longo da vida.<\/strong><\/p>\n\n\n\n<p>A decis\u00e3o envolve n\u00e3o apenas ci\u00eancia, mas tamb\u00e9m vontade pol\u00edtica e compromisso com a equidade.
