N\u00e3o basta saber o valor de refer\u00eancia; \u00e9 preciso entender o mecanismo entre as paratireoides, os rins, os ossos e o intestino.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-paratormonio-pth\"><span class=\"ez-toc-section\" id=\"Paratormonio_PTH\"><\/span><strong>Paratorm\u00f4nio (PTH)<\/strong><span class=\"ez-toc-section-end\"><\/span><\/h2>\n\n\n\n<p>Tudo come\u00e7a com a detec\u00e7\u00e3o. As paratireoides possuem sensores chamados <strong>CaSR<\/strong> (receptores sensores de c\u00e1lcio). Quando o n\u00edvel de c\u00e1lcio no sangue cai, esses receptores percebem a varia\u00e7\u00e3o e estimulam a secre\u00e7\u00e3o de PTH.<\/p>\n\n\n\n<p>O PTH \u00e9 o grande horm\u00f4nio hipercalcemiante e atua em tr\u00eas frentes principais para &#8220;resgatar&#8221; o c\u00e1lcio:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Nos Ossos:<\/strong> Ativa os osteoclastos, aumentando a reabsor\u00e7\u00e3o \u00f3ssea. Isso retira c\u00e1lcio da matriz \u00f3ssea e o joga na corrente sangu\u00ednea.<\/li>\n\n\n\n<li><strong>Nos Rins (A\u00e7\u00e3o Direta):<\/strong> Aumenta a reabsor\u00e7\u00e3o renal de c\u00e1lcio (trazendo-o de volta para o sangue) e, em contrapartida, estimula a excre\u00e7\u00e3o de fosfato.<\/li>\n\n\n\n<li><strong>Nos Rins (A\u00e7\u00e3o Indireta\/Enzim\u00e1tica):<\/strong> Estimula a enzima <strong>1-alfa-hidroxilase<\/strong>. Vamos seguir o caminho dela:<\/p>\n\n\n\n<ol class=\"wp-block-list\">\n<li><strong>Origem:<\/strong> A principal fonte \u00e9 a pele, onde a radia\u00e7\u00e3o UV converte o 7-desidrocolesterol em <strong>colecalciferol<\/strong> (ou 25-OH-Vitamina D). Tamb\u00e9m a obtemos pela dieta na forma de ergocalciferol (Vitamina D2).<\/li>\n\n\n\n<li><strong>No F\u00edgado:<\/strong> A vitamina sofre a primeira hidroxila\u00e7\u00e3o, virando 25-OH-Vitamina D3.<\/li>\n\n\n\n<li><strong>Nos Rins (A Ativa\u00e7\u00e3o):<\/strong> Aqui acontece a m\u00e1gica. Fonte: Acervo de aulas do Grupo MedCof.\u00a0<\/figcaption><\/figure>\n\n\n\n<p><strong>As fun\u00e7\u00f5es da Vitamina D ativa (1,25-OH2-D):<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Intestino (TGI):<\/strong> Aumenta a absor\u00e7\u00e3o tanto de c\u00e1lcio quanto de fosfato.<\/li>\n\n\n\n<li><strong>Ossos:<\/strong> Atua na reabsor\u00e7\u00e3o e forma\u00e7\u00e3o \u00f3ssea.<\/li>\n\n\n\n<li><strong>Paratireoide:<\/strong> Realiza o feedback negativo, suprimindo a produ\u00e7\u00e3o de PTH quando os n\u00edveis est\u00e3o adequados.<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-o-transporte\"><span class=\"ez-toc-section\" id=\"O_Transporte\"><\/span><strong>O Transporte<\/strong><span class=\"ez-toc-section-end\"><\/span><\/h2>\n\n\n\n<p>Essa \u00e9 uma pegadinha cl\u00e1ssica de prova e de plant\u00e3o.
