nosso pa\u00eds, especialmente se voc\u00ea est\u00e1 no plant\u00e3o da pediatria, \u00e9 a queixa de arranhaduras ou mordeduras de felinos.<\/p>\n<h2><span class=\"ez-toc-section\" id=\"Doenca_da_Arranhadura_do_Gato_Introducao\"><\/span><strong>Doen\u00e7a da Arranhadura do Gato: Introdu\u00e7\u00e3o<\/strong><span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>A Doen\u00e7a da Arranhadura do Gato (tamb\u00e9m conhecida como Linforreticulose de inocula\u00e7\u00e3o ou benigna, Linfadenite regional bacteriana ou Febre da arranhadura do gato) \u00e9 uma<strong> doen\u00e7a infecciosa causada pela bact\u00e9ria Bartonella henselae.<\/strong><\/p>\n<p>\u00a0<\/p>\n<p>Sendo assim, ela afeta principalmente<strong> indiv\u00edduos arranhados, mordidos ou lambidos por gatos.<\/strong> Desse modo, a DAG \u00e9 uma infec\u00e7\u00e3o zoon\u00f3tica comum em todo o mundo, e entender sua etiologia, manifesta\u00e7\u00e3o cl\u00ednica, diagn\u00f3stico, tratamento, preven\u00e7\u00e3o e profilaxia \u00e9 crucial para o manejo eficaz.<\/p>\n<h2 style=\"font-family: Poppins, sans-serif; color: #7a7a7a;\"><span class=\"ez-toc-section\" id=\"Microbiologia\"><\/span><strong>Microbiologia<\/strong><span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>A ocorr\u00eancia da DAG se deve \u00e0 <strong>bact\u00e9ria Gram-negativa Bartonella henselae, encontrada principalmente em gatos<\/strong>. Dessa forma, a transmiss\u00e3o da bact\u00e9ria aos humanos acontece atrav\u00e9s de arranh\u00f5es ou mordidas de gatos infectados ou pelo contato com sua saliva. Isso pode ser seguido por linfadenopatia regional dolorosa, ocorrendo tipicamente uma a tr\u00eas semanas ap\u00f3s a les\u00e3o inicial.<\/p>\n<figure>\n\t\t\t\t\t\t\t\t\t\t<img loading=\"lazy\" decoding=\"async\" width=\"371\" height=\"278\" src=\"https:\/\/www.grupomedcof.com.br\/blog\/wp-content\/uploads\/2023\/05\/CSDtypicalpapularlesion.jpg\" alt=\"\" srcset=\"https:\/\/www.grupomedcof.com.br\/blog\/wp-content\/uploads\/2023\/05\/CSDtypicalpapularlesion.jpg 371w, https:\/\/www.grupomedcof.com.br\/blog\/wp-content\/uploads\/2023\/05\/CSDtypicalpapularlesion-300x225.jpg 300w\" sizes=\"auto, (max-width: 371px) 100vw, 371px\" \/><figcaption>Fonte: Uptodate, 2023<\/figcaption><\/figure>\n<p><strong>Os linfonodos afetados ficam aumentados, sens\u00edveis e podem supurar<\/strong> (minoria dos casos), as cadeias mais afetadas s\u00e3o axilar, cervical e inguinal.<\/p>\n<figure>\n\t\t\t\t\t\t\t\t\t\t<img loading=\"lazy\" decoding=\"async\" width=\"212\" height=\"324\" src=\"https:\/\/www.grupomedcof.com.br\/blog\/wp-content\/uploads\/2023\/05\/EnlargedlymphnodeinCSD.jpg\" alt=\"\" srcset=\"https:\/\/www.grupomedcof.com.br\/blog\/wp-content\/uploads\/2023\/05\/EnlargedlymphnodeinCSD.jpg 212w, https:\/\/www.grupomedcof.com.br\/blog\/wp-content\/uploads\/2023\/05\/EnlargedlymphnodeinCSD-196x300.jpg 196w\" sizes=\"auto, (max-width: 212px) 100vw, 212px\" \/><figcaption>Fonte: Uptodate, 2023<\/figcaption><\/figure>\n<p>Al\u00e9m disso,<strong> outros sintomas podem incluir febre, fadiga, dor de cabe\u00e7a e mal-estar.<\/strong> Entretanto,<strong> em casos raros, podem ocorrer complica\u00e7\u00f5es<\/strong> como envolvimento neurol\u00f3gico, hepatoesplenomegalia ou manifesta\u00e7\u00f5es oculares ( S\u00edndrome \u00f3culo-glandular de Parinaud).<\/p>\n<p>\u00a0<\/p>\n<p>Em pacientes imunocomprometidos, a linfadenite pode persistir por algumas semanas e involuir espontaneamente.<\/p>\n<h2><span class=\"ez-toc-section\" id=\"Doenca_da_Arranhadura_do_Gato_Diagnostico\"><\/span><strong>Doen\u00e7a da Arranhadura do Gato: Diagn\u00f3stico<\/strong><span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p><strong>O diagn\u00f3stico \u00e9 eminentemente cl\u00ednico!<\/strong> Para pacientes que apresentem as manifesta\u00e7\u00f5es cl\u00ednicas associado ao contato pr\u00e9vio com gatos, o diagn\u00f3stico pode ser estabelecido.<\/p>\n<p>\u00a0<\/p>\n<p>\u00a0<\/p>\n<p>Deve-se considerar em casos de linfadenite cr\u00f4nica a <b>an\u00e1lise histol\u00f3gica do linfonodo.<\/b> Alguns testes sorol\u00f3gicos e de PCR podem identificar a Bartonella henselae, mas s\u00e3o <b>poucos dispon\u00edveis em nosso meio.<\/b><\/p>\n<h2 style=\"font-family: Poppins, sans-serif; color: #7a7a7a;\"><span class=\"ez-toc-section\" id=\"Tratamento\"><\/span><strong>Tratamento<\/strong><span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>Dessa forma, maioria dos casos, a DAG \u00e9 uma<b> doen\u00e7a autolimitada que se resolve espontaneamente sem tratamento espec\u00edfico. <\/b>No entanto, em casos graves ou complicados, a antibioticoterapia pode ser necess\u00e1ria.<\/p>\n<p>\u00a0<\/p>\n<p>\u00a0<\/p>\n<p>Portanto, no tratamento da DAG, <b>costuma-se utilizar antibi\u00f3ticos <\/b>como azitromicina e doxiciclina.<strong> A dura\u00e7\u00e3o do tratamento depende da gravidade<\/strong> da doen\u00e7a e da presen\u00e7a de complica\u00e7\u00f5es, mas geralmente ocorre dentro de 14 dias.<\/p>\n<p><strong>&#x1f6a8; ESQUEMA TERAP\u00caUTICO<\/strong><\/p>\n<ul style=\"font-size: 16px; background-color: #ffffff;\">\n<li style=\"font-size: 16px;\">1a escolha: Azitromicina 500mg VO 1x\/dia por 14d<\/li>\n<li style=\"font-size: 16px;\">Claritromicina 500mg VO 12\/12h por 14d<\/li>\n<li style=\"font-size: 16px;\">Doxicilina 100mg + ciprofloxacino 500mg VO 12\/12h por 14d\u00a0<\/li>\n<\/ul>\n<p>Por fim, lembre-se de fornecer um bom tratamento de controle da dor para o paciente, visto que<strong> a linfadenite \u00e9 bastante dolorosa<\/strong>.
